How do I know if my animal is an EC carrier?
Behind the complicated name "E. Cuniculi" - Encephalitozoonosis, Encephalitozoon Cuniculi - hides a pathogen that often leads to a disease in the nervous system of rabbits. These are fungus-like single-celled organisms that nest within the animal's cells. First things first - some studies show that about 80% of all pet rabbits are carriers. Other studies speak of about 41-45% of pet rabbits in Germany being affected by EC as carriers. E. Cuniculi is transmitted from the mother animal even before birth. Previously healthy rabbits also ingest the pathogen orally through contaminated food, as sick animals excrete spores in their feces and urine. The affected rabbits are latently infected. Detection is carried out by determining the antibodies against the pathogen (colloquially: determining the "titer"). For this, the veterinarian takes a blood sample from the rabbit and sends this blood sample to a laboratory. Two different testing procedures can be applied. The test result is available within a few days. If the rabbit has recently had a so-called EC flare-up, the titer on the finding is significantly elevated compared to the baseline value. Now, a positive test result does not necessarily indicate an acute illness. It is important to know that about 50% of all rabbits test positive due to antibodies. Conversely, rabbits that were infected in the womb do not have antibodies, as the parasites are not recognized as "foreign." In principle, EC treatment should be started without waiting for the lab result. The blood result merely serves to substantiate the suspicion of EC.
Triggering Factors
Certain factors can promote an EC flare-up and have been identified as triggers. First, stress factors must be mentioned. Immediately after a pairing, EC flare-ups occur significantly more frequently. The loss of a partner animal, relocation of the enclosure, or poor housing conditions (uncleanliness in the enclosure, limited space, poor nutrition...) increase the likelihood of getting an EC flare-up. The immune system plays a central role - rabbits with a stable immune system can keep the pathogen under control, so that no clinical symptoms occur.
What are the symptoms?
An EC flare-up can manifest itself in many different ways. Affected rabbits usually have only one of the following possible symptoms:
- The most well-known is paralysis of the limbs. The rabbit loses strength in its legs and drags its hind limbs as if paralyzed. The animals often only crawl forward with their front legs. Unilateral paralysis and paralysis of the head or front legs also occur.
- Furthermore, a frequent symptom is a head tilt. The rabbit often runs disoriented in circles or rolls on the ground.
- Twitching eye movements are also a classic indicator.
- The rabbits can sit lethargic, immobile and stare blankly.
- Uncontrolled incontinence occasionally occurs as a clinical picture.
- Visible signs can result in uveitis, a hemorrhage/deposit in the eyes.
- The kidneys are often affected in long-term EC patients, which manifests in a blood count with poor kidney values. Also, the heart, digestive organs, lungs, and liver can be affected.
- Noticeably frequent head shaking has been reported as a symptom.
- Unexpected and unfounded panic attacks can occur.
- Chronic digestive problems, especially constantly recurring bloating, can be caused by E. Cuniculi.
My rabbit shows the mentioned signs - what should I do?
Go immediately to a veterinarian experienced with rabbits. In the case of an EC flare-up, it is important that the mentioned medications are administered within hours. Do not wait a minute longer! Every unused unit of time significantly delays the healing process. The veterinarian will usually prescribe the following medication:
- 30 days Panacur
- 10 days Baytril
- 10 days Vitamin complex (Vitamin B1, B6, B12)
You will receive the exact prescription of the medications and the dosage from your veterinarian!
If the veterinarian is unsure in their diagnosis and wants to euthanize the animal immediately - leave the practice with the rabbit and get a second opinion from a veterinarian specializing in rabbits. From our own painful experience, we can say that inexperienced veterinarians have carelessly euthanized former protégés of ours due to an EC flare-up. Unfortunately, as a former foster home, we only found out about it later. Our experience shows that we were able to get every rabbit with an EC flare-up back on its feet within a few weeks. This requires patience (on average about 3 - 12 weeks with quick diagnosis), a lot of love, a safe environment, and targeted medication. If the medication is administered promptly after the EC flare-up, an improvement often shows within a week. Treat the animal according to the doctor's dosage instructions and present the animal again after the treatment plan has expired. Avoid too frequent trips to the vet "in between," as these can act as stressors and negatively affect recovery.
What do I do with the partner animals during the treatment period?
Do not separate the sick animal! The sick animal now needs the love, care, and affection of its companions. If the companions attack the sick animal, separation should be considered. However, this is rarely to be expected. Treat all cohabitants preventively, as you can assume that all contact animals are also EC carriers. In the case of significant coordination disorders, protective measures must be taken to prevent the rabbit from injuring itself on objects. Set up a safe and senior-friendly enclosure for the rabbit that is well-padded.
What kind of food should I offer and how?
Distribute the food in the enclosure in such a way that the rabbit can eat it without effort. For severe motor impairments, low bowls and dishes are recommended. If the animal cannot drink water on its own, a feeding syringe can help. Offer fresh tea or heavily diluted sugar-free juice so that the animals can choose. The feeding syringe with Herbicare or Critical Care is also used if the animal cannot eat on its own in the first few days. Additionally, with the veterinarian's approval, infusions can be given under the skin to prevent dehydration and support kidney function. The energy requirement of an EC-affected animal is increased - you can give treats! In addition, a healthy and species-appropriate diet with meadow grass, fresh herbs, vegetables, and fruit is recommended. Nettles (also as tea) and ribwort plantain are well received. Yarrow and broadleaf plantain can also be offered, preferably fresh.
How do I offer water?
Especially on hot summer days, additional water intake is necessary. In a severe EC attack, the head can be severely twisted, making independent water intake impossible. Administering water two to three times a day with a feeding syringe is useful. We had success with a puppy bowl. The rabbits can drink without risking drowning in the water.
Remember the eyes
In the worst case, with a twisted head, an eye rubs against the floor. Remember to take good care of the eye. For corneal injuries, the veterinarian will prescribe an appropriate ointment. Otherwise, keep the eye clean and remove foreign bodies in time.
What hygiene measures need to be taken?
During the treatment period, increased hygiene measures must be taken. Change towels & blankets daily and boil them in the washing machine. Alternatively, laundry disinfectant can be added to the laundry.
Are there any residual damages?
It is important to know that a relapse can occur at any time. Therefore, always keep Panacur and the vitamin complex stocked at home. The good news is that none of our foster animals affected by EC suffered any permanent damage. This is certainly related to the fact that a flare-up was recognized and treated promptly. Even if the treatment takes several weeks, you can expect a significant improvement in the situation. If the EC disease is not recognized immediately and the head remains tilted, always remember that the rabbit still experiences a lot of joy in life. Do not try to straighten its head. The nerve structures are and remain damaged. False pity is inappropriate here. The animal should not be euthanized solely due to an acute EC flare-up! The disability does not impair the animal's quality of life - it will quickly get used to the head position and play with its companions as before.
How do I get further advice from other affected people?
Most rabbit welfare organizations have an EC specialist who can provide in-depth advice. In addition, there is a Facebook group on the topic of EC, which provides good counseling.
Should I have my pet tested for EC?
To determine a past or acute infection with Encephalitozoon cuniculi (EC) in rabbits, there are two different main testing procedures:
🧪 1. Serological test (antibody detection)
Objective: Detection of antibodies against E. cuniculi in the blood
Method: Usually using ELISA or immunofluorescence test (IFAT)
Significance:
A positive result indicates that the rabbit has had contact with the pathogen (acute or in the past). A high titer can indicate a current infection, but is not conclusive!
Advantages: Fast and relatively inexpensive
Disadvantages: No distinction between active and resolved infection
🔬 2. Direct pathogen detection (e.g. PCR)
Objective: Detection of E. cuniculi DNA in urine, cerebrospinal fluid (CSF) or tissue
Method: Polymerase Chain Reaction (PCR)
Significance: A positive PCR test in urine indicates active shedding and thus a current infection. The test is helpful for neurological symptoms in combination with CSF analysis.
Advantages: Clear indication of active infection
Disadvantages: E. cuniculi is not constantly shed → false-negative results possible
The two tests show different stages of infection - they can be temporally decoupled. A positive serology result therefore does not indicate whether shedding is actually occurring at the time of PCR - and vice versa.
A combination of both tests (serology + PCR) is useful to confirm the diagnosis - especially for non-specific symptoms such as head tilt, paralysis or kidney problems. In these cases, the testing procedures actually make sense.
What argues against a test?
For a short time now, EC tests have been used for partner selection in asymptomatic animals.
It should be known that the results of serology and PCR are not necessarily congruent. If one decides on a test, one should be aware of the susceptibility to errors of both test procedures.
a. PCR often false-negative
- E. cuniculi is not continuously shed, but intermittently.
- A negative PCR test in urine therefore does not rule out an infection.
This means: Even with an actual disease, the PCR can be false-negative.
b. Positive serology ≠ active disease
- Many rabbits (up to 80%) have antibodies against EC - without clinical symptoms.
- A positive antibody test alone does not prove active disease or EC as the cause of symptoms such as head tilt.
It is important to know that a negative test result does not make a statement about the presence of an infection! The rabbit can be a "carrier". It was infected early with mother's milk and lives without flare-ups. The negative test result confirms that the tested animal has not experienced an active infection so far. Neither procedure confirms that the tested rabbit is actually EC-negative - not even with a negative test result!
The rabbit could be EC-positive, have an active outbreak, but no E. cuniculi pathogens were shed at the time of testing and thus not detected by the EC test. In this case, you will also receive a false negative test result.
The combination of serology and PCR only makes sense in very specific situations - e.g. to differentiate between acute vs. resolved infection. In most cases, however, it is neither clear nor necessary and tends to lead to uncertainty in interpretation.
The significance of EC tests for selecting a new partner is limited and does not provide a clear indication of whether the animal is suitable or not due to an EC infection. The combination of serology and PCR for the determination of Encephalitozoon cuniculi (EC) makes little sense in the context of partner selection or the socialization of a new rabbit because the tests do not provide reliable information on whether an infection can be avoided or prevented.
Why serology + PCR are not very helpful for assessing a new rabbit
1. Serology says nothing about the risk of infection
A positive antibody test (serology) only shows that the animal had contact with the pathogen at some point.
Even completely healthy animals without active disease often show antibodies.
Conversely, a seronegative animal can certainly be infected, but still in the incubation period or immunosuppressed.
👉 Result: Serology does not help to assess whether the new animal is currently infectious or at risk.
2. PCR is often false-negative
EC is only intermittently shed through urine - not continuously.
A single negative PCR means nothing - even infected animals shed nothing at times.
👉 An animal can be infected AND infectious, even if the PCR is negative.
3. High prevalence in the population
Depending on the region and husbandry, up to 80% of pet rabbits are seropositive - separating them into "EC-free" vs. "EC-affected" is hardly realistic.
Even in breeding facilities or animal shelters, it is practically impossible to reliably identify EC-free animals.
4. Stress of separation and selection is counterproductive
The idea of selecting "suitable" or "safe" animals through tests often leads to:
- unnecessary stress through quarantine
- exclusion of well-suited animals
- false sense of security
Instead, the focus should be on careful socialization, good hygiene, and strengthening the immune system.
